Hello,
Judging by external characters the fly in question should be Sapromyzosoma quadricincta, but the genitalia don't fit. At least I can't make them fit the drawing in Shatalkin. There's a kind of resemblance though ...
It has a pair of black spots on T6 and three preapicals on t2 (one dorsal and two ventrals) and only one of the ocellar bristles is located inside the ocellar triangle, curiously enough.
If it isn't quadricincta then what are the options?

Most species show considerable variation within the basic structure of their genitalia. This is
quadricincta but the direction of the spines on the inner side of the left lobe may vary, the thickness of the inner side of the right lobe may vary and there is some variation in the relative legnths of the inner and outer side of the lobe.
